2. Report of the 40th International Danube Neurology Symposium by Professor O. Bajenaru

 (Sinaia, Romania,  21-24 May, 2008)

“The 40th International Danube Neurology Symposium was organized in Romania between 21-24th May 2008, in Sinaia, in conjunction with the  6th Congress of the Romanian Society of Neurology.

Romania is a member of the Danube Neurology Association from the very beginning, and has been waiting for many years the opportunity to organize again this scientific meeting. The initiative was renewed in 2003, when the new president of the Romanian Society of Neurology, Professor Ovidiu Bajenaru sent the detailed proposal for the meeting, and was finally accepted by the Danube Nerurology Board in 2006 in Ljubljana.

Over 950 participants were registered in Sinaia, and among them more than 300 were from other countries than Romania (Hungary, Austria, Bulgaria, Croatia, Slovenija, Moldavia). The scientific program was very attentively selected by the organizing committee (Prof. Ovidiu Bajenaru, Prof. Cristina Tiu and Dr. Bogdan Popescu) in collaboration with the Danube Neurology Board, with the very active support from Prof. László Vécsei and Prof. Franz Gerstenbrand.

The Honorary Conference “Gheorghe Marinescu”, held yearly in the memory and honour of the world famous neurologist who is the founder of Romanian scientific neurology was given this year by one of the most outstanding personalities in international neurology - Prof. Vladimir Hachinski,  vice-president of the World Federation of Neurology.

Other  scientific personalities as Wolfgang Grisold ( President of the Neuroboard in UEMS ), Daniel Truong, Natan Bornstein, Amos Korczyn, László Vécsei, Riccardo Sofietti, Fabrizio Stocchi, Zvezdan Pirtosek, Zvi Ram, Dieter Schmidt, Francois Alesch, Dimitri Karussis, Peter Riederer, Anna Czlonkowska, Ariel Miller, Ioan Pascu, Cristian Dinu Popescu, Sanda Magureanu, J. Opara, S. Baloyannis, H. Binder, Gerhard Ransmayr, Emil Toescu, Daniel Bereczki, Maja Relja, K.Reiners, Christian Scherfler, H. Kingma and many others raised to high standards the scientific level of this meeting.

A teaching course moderated by Professor Michael Davidson was dedicated to “evidence-based medicine”- advantages and limits. Plenary sessions on important topics like Dementia and Movement Disorders and parallel scientific sessions have been organized, which covered hot topics as Stroke, Multiple Sclerosis, Epilepsy, Neuro-Ethics, Neurooncology, Neurorehabilitation, Peripheral Neuropathies, Myastenia gravis, Neuroprotection together with workshops and scientific symposia were just a part of the interesting program of the meeting.

The Romanian Society of Neurology is especially interested in promoting the young neurologists, so we made efforts to offer our support also for the Meeting of the YNT Organization.

More than 120 posters were submitted, out of which the best 3 were awarded prizes offered by the Romanian Society of Neurology.

The location in the environment of a beautiful mountain resort and historical old city, former summer residence of the kings of Romania and the richly decorated rooms of the Conference Center, together with the friendly ambiance contributed to the pleasant atmosphere of this meeting.

We hope that the 40th International Danube Neurology Symposium met the expectations of all participants and generated the interest to actively participate in further Danube Neruology Symposia and proved once again the important contribution which Romanian neurologists had and have in the European neurology.

3. „The Management of Multiple Sclerosis in Danubian countries” (May 23rd  2008, Sinaia, Romania)

During the 40th International Danube Neurology Symposium, a workshop in the form of a Round-Table Discussion under active participation of the representatives of Danubian countries took place. Chairpersons of the event were: Professors C. Tiu (Romania) and L.. Vécsei (Hungary).

Excellent presentations reflected about the situation of MS-Centers in regions of the Danube countries under following aspects:

I.              Medical staff of the Center (full-time MD-s, full-time and part-time nurses,

II.             Diagnositc possibilities available at the institution

III.            Data about in- and outpatient activities

IV.           Data about the MS patients (catchment area, criteria of diagnosis, etc.)

V.            Treatment of MS patients (excluding or including trials)

VI.           Important information about the Center (involvement in phase II-III trials/present a list if possible/ diagnostic and treatment difficulties, research activity, etc.)

VII. List of research activity, recent publications (max. 10).

The International Danube Neurology Association in cooperation with the University fo Szeged, Department of Neurology is planning to make a publication containing these presentations in order to offer further information about the Management of MS-Centers in the Danube-countries.

4. Progress in Multiple Sclerosis, Epilepsy, Headache and Vertigo – Teaching Course (Kazimierz Dolny, Poland, 7-9 April, 2008)

It is already becoming a tradition, that every year a Teaching Course is being organized by Professor Z. Stelmasiak in Kazimierz Dolny, Poland under the auspices of the International Danube Association for Neurological Sciences and Continuing Education, in collaboration with Lublin Branch of Polish Neurological Society and the Department of Neurology Medical University of Lublin. The event was under the partonage of Andrzej Ksiazek, Rector of the Medical University of Lublin.

The Teaching Course took place from 7-9 April, 2008 and the topics were widened with Vertigo, so the event was called Progress in Multiple Sclerosis, Epilepsy Headache and Vertigo Teaching Course. Professor Stelmasiak, Chairman of the Organizing Committee  expressed in his welcome address in the Final Programme, „Like in recent years, the scientific programme focuses on progress in diagnostic and treatment of multiple sclerosis, epilepsy and headache. This year, for the first time, scientific programme will be enlagred by new vertigo topic.

The Organizing Committee hopes that this meeting will be equally successful event as it happened previously. The highest educative and scientific level of this conference is guaranteed by the attendance of many internationally recognised experts.“

The Presidential Lecture was held by Professor F. Gerstenbrand (Austria) with the title: Apallic Syndrome/Vegetative State, clinical aspects in diagnosis and treatment, special care of this severest neurological condition.

Furthermore, the event was followed by different Danube Lectures on Multiple Sclerosis, Epilepsy, Heachache and Vertigo:  Professor H.P. Hartung: (Germany): Progress in MS, Professor D. Muresanu (Romania): Neuroprotection and Neuroplasticity in MS, Professor G. Bauer (Austria): Status epilepticus and Professor L. Vécsei (Hungary): Recent developments in understanding of pathomechanism of migraine and treatment strategies. The Danube Lectures were followed by numerous excellent presentations. The final programme with the 42 lectures were published in a bilingual (English-Polish) Abstract Book.

It can be declared that the meeting was again successful with numerous participants, speakers and honorary guests.

Again, just like in former years, we would like to express our cordial thanks to Professor Stelmasiak, the Department of Neurology, Medical University of Lublin and the local organizers for this successful meeting!

5. Report of “The Sixth Congress of the  International Society for the Amelioration of the Quality of Life in Chronic Neurological Conditions”. By Professor S.J. Baloyannis (Marseille, France, 26-30 January, 2008)

“’The Sixth International Congress of the Improvement of the Quality of Life in Chronic Neurological Conditions’ was organized by the International Society for the Amelioration of the Quality of Life for Chronic Neurological Conditions. The congress was held in Palais du Faro, in Marseille, France from 26th to 30th of January 2008. Honorary President of the congress was Prof. Franz Gerstenbrand and President Prof. Dr. Stavros J.Baloyannis.

Τhe congress was mostly focused on Dementia, Parkinson’s disease, Epilepsy and MS. The international scientific committee in collaboration with the organizing committee has prepared an exciting scientific program especially targeted on new concepts in the pathogenetic background of the chronic disease and the transfer of new data from the research into the clinical practice for improving the beneficial contribution to the patients.

Outstanding speakers have been invited from many countries in order to release their knowledge, experience and personal view on the rapidly expanded fields of genetics, pathophysiology, neuropathology, neuroimmunology, neuroradiology, neuropharmacology, therapeutics, radiating at the same time the enthusiasm for the research, the ethics and the devotion to the crucial field of the chronic neurological diseases. A special effort has been produced by the Organizing Committee in order to promote young neuroscientists to participate with platform presentations and posters on the main topics of the congress as well as with free communications.

The congress was attended by 200 delegates from 12 countries. Leading scientists in the field of dementia, epilepsy, multiple sclerosis, neuropathology, neuroradiology, neurochemistry, neuroimmunology, and medical ethics released lectures and presented platform and poster presentations. The congress included 20 plenary lectures, 11 oral communications, 40 posters and one round table.

The opening ceremony was held in the Papal Palace in Avignon, where an excellent performance of Chamber Music by privileged young artists was held in the Conclavium.

The main lecture was released by Prof. Gerstenbrand entitled “The right and the ethical obligation for a care in severest and hopeless neurological conditions” which attracted much attention of the audiences and stimulated long and very fruitful discussion.

The round table on the” End of the Life” was chaired by Prof. Gerstenbrand, with the following speakers: F. Gerstenbrand, K. Jellinger, A. Korczyn and SJ Baloyannis. Many aspects from the medical, economical, social and spiritual point of view were discussed and enforced the participation of audiences who posed many crucial questions.

All the contributions lectures, oral presentations and posters were excellent and mostly focused on the basic research, clinical investigation, diagnostic procedures and treatment and management of patients suffered from chronic neurological disease, with the goal to ameliorate the quality of life of the patients and to through light, hope and perspectives in their expectations.

The effort for the amelioration of the quality of life in chronic neurological diseases is in fact the third way or the third leg of Neurology, besides the treatment of the Acute Neurological Conditions and the Neurorehabilitation.

The International Society for the Amelioration of the Quality of Life is under the Auspices, support and blessings of the Ecumenical Patriarch of the Orthodox Church and is based on a strong ethical, humanitarian and ecumenical philosophical background.”

6. European Board Examination in Neurology – a Joint Task of the UEMS/EBN, the ENS and the EFNS

By Professor  Wolfgang Grisold and Professor Svein Mellgren on behalf of the UEMS/European board of neurology

“The UEMS/Board and Section of  Neurology will establish a European Board Examination in neurology from 2009 onwards. This will be part of a European quality circle to improve and boost the training and education of neurology in Europe. Additionally the UEMS/EBN will establish a visitation service of neurological departments by a UEMS/EBN visitation committee to secure the quality of education (information UEMS website).

The European examination will consist of two parts. A written part is based on a multiple choice test and a standardized oral examination. This will consist  of  4 cases with structured questions and answers. Additional points can be obtained by giving an own case presentation or  presenting scientific work achieved during training. The question  and  the case pool will be developed with both the scientific panels of the European Neurological Society (ENS) and the European Federation of Neurological Societies (EFNS), which together with the UEMS/EBN will create a joint European Examination Committee.

As most European countries have their own qualification systems, the UEMS /EBN examination will have no legal power and can only be considered as a significant mark of excellence. For the future it is hoped that countries which have no qualification system yet, will adopt this examination as their standard in neurological training assessment. The successful candidates of the UEMS/EBN will be awarded the title “Fellow of the European Board of Neurology”.

The details and registration procedure will be announced on the UEMS/EBN website (http://www.uems-neuroboard.org/ebn). In the first phase of the examination only candidates from EU countries are eligible. They must also have either passed their national examination or have a confirmation of their national society that they eligible to do so. This national confirmation will also have to take into account the practical skills acquired during training. Thus, the UEMS/EBN examination will be a three step process consisting of national certification, and the written and oral UEMS/EBN test.

The first examination will take place during the ENS congress in Milan in 2009.”

7. ORPHEUS: Short History, Mission and Visions. By Professor Zdravko Lackovic, President of ORPHEUS – Organisation for PhD Education in Biomedicine and Health Sciences in the European System (University of Zagreb School of Medicine, Croatia)

“ORPHEUS is a European association of PhD programmes in biomedicine and health sciences initiated at the Second  European  Conference in Zagreb 2005.  It followed the First Conference, held in April. The  outcome of the Conference was the “Zagreb Declaration” which is in a way a unique document defining for the first time PhD programme.

This Declaration is up to now supported by representatives of more than 70 medical schools. The establishment of ORPHEUS comes at the time when according to “Bologna process” PhD programme should be established as a third circle of high education, but also in a time when some countries are without PhD in medicine while other have two some perceive PhD programme as a research based only while other look at it as a prolonged learning. ORPHEUS comes also at the time when the differences in the medical publications visibility on our computer screen (in PubMed) of former “East” vs. “West” European countries are enormous.

With so huge differences in research input and output - to achieve harmonization, mobility, and other noble goal of “Bologna process”, according to our opinion, requires new ideas, new organisation, new visions - or Europe as a society based on knowledge (and health) will remain only a dream.  Support as one provided at Danube Neurology Conference 2008 is thus of utmost importance.”

8. Establishment of collaboration between the Cochrane Neurological Network and the European Association of Young Neurologists and Trainees. By Dr. Lászlo Sztriha, Secretary-elect, YNT.

“The European Association of Young Neurologists and trainees (YNT) has proposed the establishment of collaboration between the organization and the Cochrane Neurological Network, with the aim of improving the knowledge of evidence-based practices among young neurologists and neurologists in training. The Cochrane Neurological Network is an entity of the Cochrane Collaboration, and it works with neurological Cochrane Review Groups to improve their productivity and communicate the results of their research to a worldwide neurological community.

The most recent executive meeting of the Network has expressed interest towards the establishment of an official relationship between the two organizations, and also formulated some ideas to initiate collaboration. The Neurological Network regularly organizes conferences and workshops on single neurological topics (e.g. meningitis, subarachnoid hemorrhage, carpal tunnel syndrome), where Cochrane Systematic Reviews are presented together with clinical and epidemiological views and experiences.

Such future events are planned to be organized together with YNT. Furthermore, key-note presentations by Cochrane-affiliated lecturers are also considered to be given during future YNT meetings, inclusive of YNT sessions at the annual Danube Neurology meetings. The director of the Cochrane Neurological Network is Dr. Teresa Cantisani in Perugia, Italy, whereas YNT is represented in this activity by Dr. Lászlo Sztriha in Szeged, Hungary. “
